Destiny Support acknowledges that vulnerable people need the holistic support necessary to get their lives as well as gain independence.

Our range of services includes:-

• Offering floating or outreach support by mapping out locally available services which includes assisting individuals to access local colleges/community centres, connexions, job centre/benefit advice, help with filling out forms, housing, apprenticeships, meaningful occupation, health services (GPs, dentist, other related clinics), translation services due to language barrier and any other identified support.
• Ensuring that our Service Users’ accommodation/environment will be safe, secure and risk assessed.
• Working alongside Boroughs and other professional external agencies involved in their support plan which will identify their needs following through with the action plan objectives and SMART goals set.
• Managing and providing feedback on an individual's progress/activities whilst within our care.
